REPATRIATION GUIDELINES
The Embassy kindly wishes to inform citizen that submission of a Request/Application for a waiver for the repatriation of human remains should be addressed to the Honourable Minister of Health and Submitted to Port Health Services (Please note that the application should be written by the relatives of the deceased or other authorized representative).
2. Request/Application to provide information of cause of death and include the following documents:
I. A data page of the deceased international passport
II. Death certificate
III. Embalment certificate
IV. Evidence of payment of N5,161.25 ( five Thousand, One Hundred and Sixty One Naira, Twenty-Five Kobo) to the port Health Services TSA account (052102900100-Tender)
3. The waiver must accompany the request to the airline for the transportation of the human remains.
Kindly note that the NCDC guidelines for the safe transportation of remains of persons suspected or confirmed to have died of covid-19 can be accessed on the centres’s web-site at www.ncdc.gov.ng.
