PASSPORT INFORMATION

Requirements for obtaining Standard Passport

The standard passport can be obtained with any of the following reasons:

    1.  Fresh (First) Application

    2.  Re-Issuance of Passport

 

Please note that The Embassy of Nigeria Ukraine does not issue Diplomatic and Service passport and all payments for issuance of ordinary or standard passport are done online  via www.immigration.gov.ng

 

PASSPORT REQUIREMENTS

 

Applying for passport for baby:

 

        Application letter from parents requesting passport issuance

        Completed passport application form and 2 passport sized photographs (passport photographs endorsed at the back by the Guarantor);

        Completed Guarantors form duly signed.

        Marriage certificate of parents.

        Birth Certificate (certificate issued by the National Population Commission shall be required from applicants whose date of birth is with effect from Dec 1992) or Age Declaration attached with one passport-sized photograph and duly endorsed by a Commissioner for Oaths

        Acknowledgement slip and evidence of payment online ( www.immigration.gov.ng)

 

Applying for fresh passport for Adults ( Freshman):

        Application letter requesting for a passport addressed to the Ambassador or Chage d Affaires

        Completed passport application form and 2 passport sized photographs (passport photographs endorsed at the back by the Guarantor);

        Completed Guarantors form duly signed

Please note that applicants are physical required to visit the Embassy for issuance of passport.

 

Applying for re-issuance of Passport for Adults:

        Completed passport application form and 2 passport sized

        Acknowledgement slip and evidence of payment online ( www.immigration.gov.ng)

        Copy of data page of  current international passport

Please note that applicants are physical required to visit the Embassy for issuance of passport.


Nigerians applying for reissuance of passport from Lithuania, Latvia, Estonia, Moldova and Georgia are to follow instructions below to receive a support letter from the Embassy for processing visa to come to Kyiv-Ukraine to process a new passport.

 

  • 1.  Application Letter addressed to the Embassy and also stating your address and phone number in the country of residence
  • 2.   Two (2) passport sized photograph.
  •    Letter of introduction of University or any institute where you study. In case of   non students, a letter from place of work or proof of business in the country of residence.
  • Copy of residence card both front and back of the card.
  •   The following document should be sent to the embassy email: nigeriaconsul@gmail.com for processing.
      
  • Please note that only after you have been issued a visa then you can proceed online @ www.immigration.gov.ng to fill your passport application and make payment online. Also all documents submitted to the email should be brought physically to the Embassy when processing a new passport.

Applying for damaged passport:

         Application Letter stating circumstances of Damage;

         Photocopies of Data Page

         The above documents should be scanned to nigeriaconsul@gmail.com  and also note that applicants should come with the booklet of the damaged passport when coming to the Embassy to receive a new passport.

 

Applying for Change of Surname due to Marriage:

 

        Application Letter

        Copy of Marriage Certificate or Declaration of Marriage. If you were married outside of Nigeria, marriage certificate should be legalized in the Ministry of Foreign Affaires of Nigeria

        Sworn Affidavit from a Magistrate or High Court of Change of Name deposed to by Applicant;

        Copy of Newspaper Publication of Change of Name (21 days after publication);

        Passport Booklet and Copy of Data Page of Passport of both partners

        Application form online ,  Acknowledgement Slip & Evidence of Payment; (www.immigration.gov.ng)


    Applying for Lost passport:


  •     Application Letter addressed to the Embassy and also stating your address and phone number in the country of residence and how the circumstances of your passport.
  •    Two (2) passport sized photograph
  •    Police report issued in Ukraine and translated in English.
  •   Copy of data page of international passport
  •   Online payment via www.immigration.gov.ng. Please it is advised that payment is done after lost Embassy receives passport approval.
  • The following document should be sent to the embassy email: nigeriaconsul@gmail.com for processing. Applicants are also to kindly note that lost passports cases are subject to approval from immigration Headquarters Abuja and only after approval has been received can an applicant visit the embassy with the hard copy submitted to the Embassy   for processing  a new passport.

EMMERGENCY TRAVEL CERTIFICATE

Emergency Travel Certificate:

This certificate is issued to Nigerians who lose their passport while abroad. This certificate helps them to return to Nigeria. An Emergency Travel Certificate is usually valid for one journey to Nigeria only and must be surrendered to immigration authorities at the port of entry.

 

  • Application letter addressed to the Ambassador or Charge d Affaires
  • Proof of citizenship (Copy of Nigerian Passport)
  • 1 Passport photo
  • Travel Itinerary to Nigeria (Flight to Nigeria must be direct)
